I. Basic provisions
The controller of personal data pursuant to Article 4(7) of Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data (hereinafter "GDPR") is MILENIUM DENT s.r.o., ID 08852073, registered office Budečská 2165/33, Vinohrady (Prague 2), 120 00 Prague.

Personal data means any information relating to an identified or identifiable natural person. The controller has no data protection officer.
II. Sources and categories of personal data processed
The personal data processed by the controller come from the contact form filled in by the user on the controller’s website.
The following data is processed: name and surname, telephone number, email address, optional note and approximate appointment date.
III. Legal basis and purpose of processing
The legal basis for processing is consent to the processing of personal data pursuant to Article 6(1)(a) GDPR.
The purpose of processing personal data is to handle the user’s enquiry and arrange an appointment.
No automated decision-making pursuant to Article 22 GDPR is carried out by the controller.
IV. Data retention period
Personal data will be processed for the period necessary to handle the enquiry, for a period of 2 years from handling the enquiry, after which it will be deleted.
V. Recipients of personal data
Recipients of personal data are persons involved in handling the user’s enquiry. Personal data is not transferred to a third country.
VI. Your rights
You have the right of access to personal data (Art. 15 GDPR), the right to rectification or restriction of processing (Art. 16 and 18 GDPR), the right to erasure (Art. 17 GDPR), the right to object (Art. 21 GDPR) and the right to data portability (Art. 20 GDPR).
You also have the right to withdraw consent to the processing of personal data.
You have the right to file a complaint with the Office for Personal Data Protection if you believe that your data protection rights have been breached.
VII. Personal data security conditions
The controller declares that it has taken all technical and organisational measures to secure personal data, in particular password-protected access to computers, antivirus protection and access to personal data only by authorised persons.
VIII. Final provisions
By submitting the form, the user confirms that they consent to the processing of personal data to the extent stated above.
The controller reserves the right to change these conditions at any time and will publish a new version on the website.
